CBD & PD Dilatation
CBD and PD dilatation are advanced endoscopic techniques used to carefully widen selected narrowed areas of the common bile duct (CBD) or pancreatic duct (PD) to improve ductal access and drainage.
Ductal narrowing, also known as a stricture, can interfere with the normal flow of bile or pancreatic secretions. During ERCP, a suitable dilating device may be used to gently widen the affected segment before or as part of further therapeutic treatment.
CBD or PD dilatation is performed selectively according to the location, severity and cause of the narrowing and may be combined with stone removal or stent placement when clinically appropriate.
Conditions
Dilatation of the common bile duct or pancreatic duct may be considered for selected ductal conditions, including:
- Benign common bile duct strictures
- Malignant biliary narrowing in selected cases
- Pancreatic duct strictures
- Selected ductal narrowing requiring endoscopic access or drainage
- Strictures requiring preparation for stent placement
- Selected complex biliary or pancreatic duct interventions
Symptoms
Symptoms depend on whether the biliary or pancreatic duct is affected. Possible symptoms include:
Biliary Symptoms
- Yellowing of the eyes or skin (jaundice)
- Upper abdominal or right upper abdominal pain
- Dark urine or pale stools
- Itching associated with impaired bile drainage
- Fever or chills when infection is present
Pancreatic Symptoms
- Upper abdominal pain that may radiate to the back
- Recurrent episodes of abdominal discomfort
- Nausea or vomiting
- Digestive symptoms associated with pancreatic disease
- Abnormal pancreatic enzyme or imaging findings
Causes
CBD and PD strictures can develop from several conditions. The underlying cause is evaluated using clinical assessment, imaging and endoscopic findings.
- Inflammatory changes associated with pancreatitis
- Benign biliary or pancreatic duct scarring
- Post-surgical or post-procedural ductal narrowing
- Pancreatic or bile duct malignancy
- Chronic pancreatitis-related pancreatic duct changes
- Other structural abnormalities affecting ductal drainage
Treatment
CBD and PD dilatation is usually performed during ERCP. After the narrowed segment is identified, an appropriate dilating device is carefully advanced to widen the ductal narrowing according to the clinical requirement.
Depending on the patient's condition, treatment may include:
- Endoscopic dilatation of a common bile duct stricture
- Endoscopic dilatation of a pancreatic duct stricture
- Controlled widening of the narrowed ductal segment
- Stone or stent removal when clinically indicated
- Biliary or pancreatic stent placement following dilatation when required
- Repeat dilatation as part of staged treatment for selected strictures
The technique and extent of dilatation are individualized according to the duct involved, the cause and severity of the narrowing, previous interventions and the intended treatment plan.
Prevention & Follow-Up
Follow-up after CBD or PD dilatation depends on the underlying disease, the response to treatment and whether a biliary or pancreatic stent has also been placed.
Helpful measures include:
- Follow the medication and dietary instructions provided after the procedure.
- Attend scheduled follow-up and laboratory testing when advised.
- Complete recommended imaging or repeat ERCP when clinically required.
- Follow the planned schedule for stent removal or exchange if a temporary stent is placed.
- Report recurrent jaundice, fever, abdominal pain, vomiting or worsening symptoms promptly.
- Seek urgent medical attention for severe abdominal pain, significant bleeding, high fever or worsening jaundice.
Ongoing care is individualized according to the cause of the ductal narrowing and the patient's response to treatment.
